Name Requirements

You must apply with your current legal name, ensuring that you respect the order of your legal name. Your current legal name must appear on both your Application and Certified Identity Confirmation forms. The name on your application must match the name on any identity document(s) that must be submitted with your application.

Name Changes

If you have undergone a name change since your previous MCCEE application:

  • You will not be permitted to change the name in your MCC-Online account as this is the name that was approved and linked to your medical diploma at the time of your previous application.

  • When you print your online MCCEE Application Form, the name will appear as it currently exists in the MCC database.

  • To record your name change with the MCC:

    • On your printed MCCEE Application Form, cross out the family and/or given name(s) to be corrected.

    • Print the current name(s) by hand in black ink and initial next to the change(s).

    • Submit a certified true copy of the original of one of the following (with translation if applicable) with your application:

      • marriage certificate
      • divorce decree
      • official court order, or
      • other legal name change document.

The MCC reserves the right to request additional documentation and confirmation as required.

Your result letters and all future documents will be issued in your current legal name. Any request to change your name on your file at a future time will require a certified legal name change document (as indicated above).
